All services rendered by Hormone Health and Wellness of The Palm Beaches are provided on a non-refundable basis once that service has been performed. This includes, but is not limited to, blood work, testing and consultation services. Customer agrees that there will not be a refund available once services are rendered and completed by Hormone Health & Wellness.

Refundable instances:

• True fraudulent card activity
• Patient pays for but never uses their consultation service and requests a refund
• Hormone Health & Wellness fails to perform the service
